6 Minute SOAP Note

My initial inspiration for creating this template came from my own experience as a therapist. I found that I was spending far too much time and energy trying to figure out what I should and shouldn't include in my progress notes, only to be left with a stack of unfinished notes and a pit in my stomach.

It felt like the majority of my hours were spent trying to catch up on documentation.
